Initial Evaluation | 60-120 minutes The first step in treatment planning involves a comprehensive assessment. On average, individual evaluations take around 90 minutes though this varies depending on an individual's specific concerns and history. Sometimes, this evaluation can be spread out across several visits and may include a separate "feedback" visit if indicated. This may involve, with your permission, contacting providers or family members who know you well and could provide a richer perspective on the concerns at hand. For children and adolescents, we meet privately with the young person, privately with the parent(s) and then come together to discuss the plan. Please bring with you to the appointment any copies of prior assessments, including neuropsychological testing and discharge summaries, and a recent medication list if medications are being prescribed. After the assessment, we will review evidence-based treatment options including combinations of psychotherapy, medications, as well as referrals to other providers to promote wellness (nutrition, fitness, and other complementary and alternative approaches to the traditional medical model).
Second Opinions and One-time Consultations | variable length Sometimes it can be helpful to have an evaluation that takes a fresh look at what is going on with respect to diagnosis and/or treatment. Other providers may seek out these type of consultations from us or you may choose to seek this out on your own. We are happy to work collaboratively with an existing treatment team to provide additional insights. We also will conduct evaluations for prescribing therapeutic service animals for conditions such as Post-Traumatic Stress Disorder and outside independent evaluations for children seeking Individualized Education Plans from their school districts. Deep Transcranial Magnetic Stimulation Consultion | 45-60 minutes
For patients considering dTMS treatment at Brooklyn Minds, this visit is a meeting to determine if deep TMS will be an appropriate and likely effective treatment, as well as reviewing risks and benefits of the treatment with one of our dTMS expert physicians.
Medication Management | 30-60 minutes If someone is already in therapy and is looking for someone to manage their psychiatric medications, we are happy to do so and to work collaboratively with the therapist. This is known as "split treatment," We subscribe to the philosophy of using the lowest dose of medication necessary and the fewest medications possible. We are often in the position of helping people come off medications that are no longer necessary or that are not helping. We incorporate recommendations regarding sleep, diet and complementary therapies and refer patients for non-pharmacologic treatments such as Transcranial Magnetic Stimulation when appropriate. We are licensed suboxone providers and prescribe for patients already on suboxone maintenance. We also frequently order pharmacogenomic testing that can help determine which drugs may be a better fit for a person's genetic makeup. While therapy can be helpful for most cases, sometimes an individual may not need therapy and in that case, we can prescribe medications without a separate therapist being involved. Every visit, regardless of whether or not someone has a primary therapist, incorporates therapeutic components but medication management visits usually occur every month or every few months and are typically 30-45 minutes in length whereas individual therapy is usually at least once per every 1-2 weeks.
"Combined" Therapy and Medication Management | 45-60 minutes Sometimes, being able to see one provider for both medication management and individual therapy makes the most sense therapeutically and practically. We can provide this when indicated. See below for a description of the types of therapy we practice. These visits typically occur weekly, although sometimes more or less frequent visits are suggested.
Individual Therapy | 45-60 minutes Like many modern therapists, we practice what is known as an "eclectic" form of psychotherapy that is tailored to meet an individual's specific needs. We have specialized training in Mentalization Based Therapy, which was designed for borderline personality disorder but can be used more broadly for many concerns regarding important relationships in one's life, from romantic partners to business partners. Dr.MacMillan also is trained in Dialectical Behavior Therapy and Dr.Muir is trained in Interpersonal Social Rhythms Therapy for Bipolar Disorder. We also have a strong foundation in cognitive behavioral therapy and more traditional psychodynamic theories. Do not worry if you have no idea what any of these things are as we will talk extensively about the various options. In order for therapy to be effective, it needs to be done at least every 1-2 weeks but we will work with you to determine what is feasible and indicated.
Group therapy | 60-90 minutes Group therapy is a powerful and often under-used form of treatment. Types of groups we may be offering include Mentalization Based Therapy groups and Young Adult Groups. Please contact us for details regarding current or upcoming offerings.
Family/Couples/Business Partner Therapy | 45-60 minutes While significant gains can be made in individual therapy, people do not exist in a vacuum and so often times involving others that are very important in one's life in the treatment makes a major impact. We utilize Mentalization Based Therapy for Families in these cases and have found that families that have difficulty tolerating other forms of family therapy find this approach to be not only tolerable but actually enjoyable and effective.
