At Brooklyn Minds, we pride ourselves on wraparound services within and outside of our immediate line of work. We recognize the importance of assisting in all areas of need in a person's life to help get them back on their feet.
Case management is broadly defined as assisting clients with non-clinical components of treatment that the client needs help with, or to promote the coordination of care.
Services that a case manager at Brooklyn Minds can provide help with can include, but is not limited to:
Navigating insurance issues
Applying for insurance
Setting up primary care appointments
Applying to schools
Vocational support (resume building, job application)
Researching additional supportive services
Assisting in daily living
The use of a case manager can be incredibly useful and we are happy to discuss with you if this service is needed or would be a helpful addition to care already being provided.
